Identify the activity by an appropriate title. Ensure you enter the location(s) of the work activity as this can significantly alter the risk; different levels of risk may arise from the same activity performed in different locations particularly if there is a difference in the standards of the facilities of the locations. Room sets can be used to quickly add multiple locations.
A brief description of the work process during which the hazardous substance(s) are used should be included here. Ensure it encapsulates the work activities to be done as well as the process factors involved.

The person carrying out the assessment must have sufficient knowledge and training to be able to carry out the assessment on behalf of the person responsible for the work. Therefore they must be competent to identify the hazards and risks, be able to select and apply appropriate risk control measures and have attended the Principles and Practice of Risk Assessment course delivered by Safety, Health and Wellbeing.
