About your work
Section 1.1 Operation/activity
Identify the activity by an appropriate title. Ensure you enter the location(s) of the work activity as this can significantly alter the risk; different levels of risk may arise from the same activity performed in different locations particularly if there is a difference in the standards of the facilities of the locations. Room sets can be used to quickly add multiple locations.
A brief description of the work process during which the hazardous substance(s) are used should be included here. Ensure it encapsulates the work activities to be done as well as the process factors involved.
Section 1.2 Person Responsible for this work
The person directing the research or work activity i.e. the Principal Investigator, Academic Supervisors, Supervisors and Line Managers are ultimately responsible for ensuring that:
- suitable and sufficient COSHH assessments are carried out,
- remain valid
- and risk control measures that are identified are implemented.
This individual is refered to as the Responsible Person (RP) in this system.
Section 1.3 Person conducting this assessment
The person carrying out the assessment must have sufficient knowledge and training to be able to carry out the assessment on behalf of the person responsible for the work. Therefore they must be competent to identify the hazards and risks, be able to select and apply appropriate risk control measures and have attended the Principles and Practice of Risk Assessment course delivered by Safety, Health and Wellbeing.
Section 1.3 will be filled automatically with the name of the author of the COSHH form.
